Goals and Objectives
During the transition year from project to integrating the additional services into ongoing programming, Heart Failure will develop an integration plan and evaluation framework. In addition, the initiative has four additional goals:

Optimizing clinical processes and operations, including work on efficient clinic set up, quality checklists and referral processes

Engaging and retaining staff and maximize their contributions, including the use of telehealth to limit travel burden

Further developing expertise through education, information sharing and development of a Provincial Heart Failure Clinic Network

Fully engaging and maximizing multi-disciplinary resources and increasing capacity so as to provide high quality of care to more Albertans with chronic heart failure
Objectives
The primary objectives for Heart Failure Initiative over the next 12 months include:

Reaching a greater number of Albertans with chronic heart failure across the province

Developing and implementing a plan to integrate Heart Failure Initiative services into existing programs

Developing a Provincial Heart Failure Clinic Network

Providing a greater opportunity for Primary Care Network and Home Care involvement

Formalizing educational materials (both for patients and health professionals)

Maximizing the role of multi-disciplinary health professionals

Ensuring quality of care provided through quality assurance measures and CCS guidelines
