Achievements
Over the last 18 months, the Heart Failure Initiative established new heart failure clinics in Grande Prairie, Wainwright, Camrose, and Medicine Hat where few or no heart failure services existed, and increased the capacity of existing clinics in Calgary, Edmonton, Lethbridge and Edmonton by expanding multi-disciplinary roles, implementing best-evidence practices, more effective triaging, and improving wait-list management and follow-up protocols.

Peace Country has opened a heart function clinic in Grande Prairie.

The Camrose Heart Function Clinic is seeing patients at the St. Mary’s Hospital in Camrose.

The Heart Failure Clinic in Wainwright has been open since June 2008 and continues to accept referrals.

In Edmonton, capacity has been increased at both the Royal Alexandra and University Hospital sites, through increases to RN positions and additional social work and pharmacy support. Work has been done to streamline access to lab work and results.

The Red Deer Heart Failure Clinic continues to grow rapidly with assistance from patient navigation.

Heart Failure Clinics in Calgary has expanded existing services, including nursing, pharmacy and social work.

In southern Alberta, Medicine Hat has opened a Heart Failure Clinic. As well, training has been provided to clinics and rural hospitals, including Milk River, Cardston, Crowsnest Pass and Taber.

Lethbridge Heart Failure Clinic has expanded through a Heart Failure Network that branches to several southern sites.
