Patient navigators provide healthcare practitioners with a single point of access to cardiac care for their patients. The navigators work to improve the coordination of services between multiple care providers, reduce duplication, help ensure patients are ready for services, and facilitate the transition of care between numerous healthcare providers.
Many cardiac services are highly specialized and thus located in the province’s two major population centres – Edmonton and Calgary. The two primary nodes for navigation are built around those services.
